MMDI:ACCESS Metro-Milwaukee DART Initiative: A Community Collaborative Effort Serving Survivors With Disabilities From Crisis to Healing S afe, Accessible & W elcoming E nvironment S urvey I. Purpose During the needs assessment MMDI:ACCESS heard from individuals and survivors with disabilities; staff and managers of the partner agencies and the community in general. Using the results of this needs assessment, MMDI:ACCESS developed a strategic plan, which included taking a close look at the accessibility, safety and welcoming nature of each partner site. To do this MMDI:ACCESS developed this survey tool and we will used the results of these surveys to create a barrier removal plan for each partner agency and site. This plan will aid the MMDI:ACCESS partners in making their facilities, as well as their programs and services more responsive to the needs of survivors with disabilities. II. Definitions Accessible Environment: An environment that can be easily entered, used or reached and is usable by as many people as possible. The collaboration believes that agencies, primarily serving survivors of domestic and sexual violence and/or abuse need to ensure programmatic, communication, physical and attitudinal accessibility; and those agencies, primarily serving people with disabilities, need to be accessible and responsive to the individuals they serve who are survivors of domestic and sexual violence and/or abuse. Both types of agencies need to also ensure that survivors with disabilities have access to community-based services, regardless of race, ethnicity, gender, sexual orientation, socio-economic status, age, disability and/or religion. Objective Finding: Undistorted by emotion or personal bias; based on observable fact (for example, the door is 36Ó wide and this was confirmed by taking the actual measurements) Safe Environment: An environment that affords protection against physical, social, spiritual, financial, political, emotional, occupational, psychological, educational or other types or consequences of failure, damage, error, accidents, harm or any other event which could be considered non-desirable MMDI:ACCESS believes that safety is a basic human right and that everyone has the right to live free of domestic and sexual violence and/or abuse. Subjective Finding: Taking place within the mind and modified by individual bias (for example, I felt safe in the office knowing that the receptionist had to buzz people through the door) Welcoming Environment: An environment that greets, receives, or entertains others cordially or hospitably Survey Team Members: MMDI:ACCESS would like to thank you for offering your time and expertise to this project. Each of you has participated in training on the use of this tool and we hope that you will find this fairly easy to use and self-explanatory. If you have questions while participating in the survey please see one of the IndependenceFirst staff members who are heading up the team. While going through the applicable sections please make note of anything you see that concerns you or that you feel is of great benefit. There is no right or wrong answers, and no two of you will likely perceive the site in the same way but we are interested in what you have to say so please feel free to elaborate as much as you need to. Once the team has completed the site survey you will meet to discuss your findings. During this meeting the team will be responsible for coming up with a plan for each site. This plan will look at all three areas (Safe, Accessible and Welcoming) equally. Once a plan is drafted an official Barrier Removal Plan will be submitted with suggestions for making the site more usable by people with disabilities who are/have experienced violence/abuse. ***All material and tools needed to conduct these site surveys will be made available by IndependenceFirst Pre-Site Survey Questionnaire (Complete one for each site) Agency: Site Location: ¥ IndependenceFirst ¥ Sojourner Family Peace Center ¥ City of Milwaukee Health Department Milwaukee ¥ County Disability Services Division Structure Type: Number of Stories: ¥ Residential/Home ¥ Office Building ¥ Apartment Building ¥ State/County/City Bldg. ¥ Other: Please complete table below for each site.
